Please read this form carefully and in full before completing the declaration at the end. By typing your name below, you confirm that you have read, understood, and freely give your consent to all statements relevant to the Service(s) you have purchased. You must complete the declaration before your order can be processed.
1. About Our Services
Limitless Health UK is a wellness, lifestyle, and performance consultancy. Our Services include blood testing and biological age testing with wellness consultations, weight cut monitoring, rapid health screening, personal training and nutrition coaching, and digital educational guides.
We are not a medical practice, clinical service, or healthcare provider. Nothing we provide constitutes medical advice, diagnosis, or treatment. We do not provide any service that is intended to diagnose, treat, monitor, or prevent disease. Nothing in our Services creates a clinician-patient relationship or any regulated clinical duty of care.
2. Your Details
I confirm that all personal information provided during the checkout process is accurate, complete, and up to date to the best of my knowledge.
3. Consent Statements
Please read each of the following statements carefully. The form is divided into sections — please read all sections and complete the declaration at the end. By signing, you confirm your understanding and agreement to all statements relevant to the Service(s) you have purchased.
ELIGIBILITY — All customers
1. I confirm that I am 18 years of age or older.
Our Services are not available to persons under the age of 18.
2. I confirm that I am resident in the United Kingdom.
3. I confirm that I am purchasing this Service for my own personal use.
4. I confirm that I am not currently experiencing any symptoms that would require urgent medical attention.
5. Where relevant to the Service purchased, I confirm that I do not have any known medical condition that would make participation unsafe without prior medical clearance.
6. I understand that Limitless Health UK provides a wellness, lifestyle, and performance consultancy service only.
No medical advice, diagnosis, treatment, or clinical assessment will be provided at any point.
7. I understand that Limitless Health UK does not provide any service that is intended to diagnose, treat, monitor, or prevent disease.
8. I understand that nothing provided by Limitless Health UK creates a clinician-patient relationship or any regulated clinical duty of care.
Our consultants are professionally qualified but act solely in a wellness capacity on behalf of Limitless Health UK.
9. I understand that any guidance, recommendations, or information provided by Limitless Health UK is general in nature and should not be relied upon as a substitute for professional medical advice.
I will seek independent medical advice from a qualified healthcare professional for any health concerns.
10. I understand that Limitless Health UK does not guarantee any specific outcome or result from its Services or recommendations.
11. I confirm that any decisions I make based on guidance from Limitless Health UK are made entirely at my own discretion and risk.
12. I understand that I have the opportunity to ask questions prior to purchasing and that I can contact Limitless Health UK directly should I wish to do so.
Sections 13–19 apply to Blood Testing, Biological Age Testing, and Rapid Health Screening purchases. If you have not purchased any of these services, please proceed to Section 20.
13. I consent to Limitless Health UK facilitating access to at-home blood testing and/or biological age testing kits provided by an independent third-party laboratory partner.
I understand that the laboratory partner independently processes my sample and that Limitless Health UK does not conduct or control the analysis.
14. I understand that blood test and biological age results are indicative only and reflect general tendencies rather than certainties.
Results do not constitute a diagnosis of any medical condition. HbA1c results do not diagnose diabetes or pre-diabetes. Testosterone results do not diagnose any hormonal condition. Cholesterol results do not diagnose any cardiovascular condition.
15. I understand that I am responsible for correctly collecting my sample in accordance with the kit instructions, including ensuring sufficient sample is collected.
For dried blood spot samples, I understand that the sample must be left to dry fully and completely before sealing and posting. Failure to follow instructions correctly may result in an invalid result. No refund will be issued in these circumstances.
16. I understand that once my sample has been posted to the laboratory, processing of that sample cannot be stopped as it falls outside Limitless Health UK’s control.
I may withdraw my consent before my sample is posted by contacting Limitless Health UK directly.
17. I understand that Limitless Health UK is not responsible for the accuracy, completeness, or timeliness of results produced by the laboratory partner, nor for any errors, omissions, or delays.
18. I consent to purchasing and using a rapid health screening test (if applicable).
I understand that rapid test results are indicative only, provide a traffic light outcome rather than exact clinical values, and are not diagnostic. A green result does not guarantee that a marker is within a clinically normal range. An amber or red result does not confirm the presence of any condition.
19. I understand that rapid tests are screening tools only and are not a substitute for laboratory testing.
I will seek independent medical advice if I have any concerns about my health.
Sections 20–24 apply to the Weight Cut Monitoring Service. If you have not purchased this service, please proceed to Section 25.
20. I consent to participating in the weight cut monitoring service provided by Limitless Health UK.
I understand this is a wellness screening and educational service only. It does not constitute a clinical assessment, medical clearance, or fitness-to-compete certificate.
21. I understand that weight cutting carries inherent physical risks including but not limited to dehydration, electrolyte imbalance, reduced cognitive and physical performance, kidney stress, cardiac strain, and in extreme cases serious medical complications.
I accept full personal responsibility for my decision to cut weight and the manner in which I do so.
22. I understand that Limitless Health UK does not supervise, direct, or control any aspect of my weight cut.
Limitless Health UK provides general wellness guidance only. The decision to train or compete remains entirely mine.
23. I confirm that all weight and urine data I provide will be self-reported honestly and accurately.
24. I understand that if any concerns arise during a check-in or data review, I will be advised to seek appropriate medical attention, and that the decision to compete remains entirely mine.
Sections 25–28 apply to Personal Training & Nutrition Coaching. If you have not purchased this service, please proceed to Section 29.
25. I consent to participating in the personal training and nutrition coaching service provided by Limitless Health UK.
I understand this is general lifestyle and fitness coaching only and does not constitute medical exercise prescription, injury rehabilitation, or therapeutic nutrition.
26. I confirm that I am physically capable of participating in an exercise programme and am not aware of any medical reason why I should not do so.
If I have any doubt, I will consult my GP before commencing any training programme.
27. I understand that nutrition guidance provided is general dietary advice only and does not constitute medical nutrition therapy.
If I have a diagnosed medical condition affecting my diet, I will consult a registered dietitian or GP before following any nutrition guidance.
28. I understand that Limitless Health UK is not liable for any injury, illness, or adverse outcome arising from participation in any exercise programme or implementation of any nutrition guidance.
Section 29 applies to Digital Guide purchases. If you have not purchased a digital guide, please proceed to Section 30.
29. I understand that digital guides produced by Limitless Health UK are for educational and informational purposes only.
They are general in nature, not tailored to individual medical needs or conditions, and are not a substitute for professional medical advice. I accept responsibility for determining their suitability for my individual circumstances.
30. I understand that any lifestyle or supplement suggestions provided by Limitless Health UK are general, non-prescriptive, and informational only.
They do not constitute a prescription or clinical recommendation. I am responsible for checking ingredients, identifying contraindications, and consulting my GP before commencing any supplement if I have a known medical condition or am taking prescribed medication.
31. I consent to Limitless Health UK collecting and processing my personal data, including my name, contact details, date of birth, and gender, for the purposes of delivering my Service.
My data will be handled in accordance with Limitless Health UK’s Privacy Policy, which I confirm I have read.
32. I consent to Limitless Health UK processing my health-related and biological data as special category data under UK GDPR, solely for the purpose of delivering my Service.
33. I consent to my results and service communications being transmitted to me via electronic means, including email.
I acknowledge that such risks exist, including the risk of unauthorised access or data interception.
34. I understand that I may withdraw my consent to the processing of my special category data at any time before my sample has been posted to the laboratory, by contacting Limitless Health UK. Once posted, laboratory processing cannot be stopped as it falls outside Limitless Health UK’s control.
35. I confirm that I have read and agree to Limitless Health UK’s Terms and Conditions of Service.

38. I understand that Limitless Health UK’s total liability shall not exceed the amount I have paid for my Service, and that participation in any of Limitless Health UK’s Services involves inherent risk.
4. Declaration & Digital Signature
| DECLARATION By providing my full name and purchasing from Limitless Health UK, I confirm that: • I confirm that I have read this entire form carefully, understand all sections, and agree to all statements relevant to the Service(s) I have purchased • I freely and voluntarily give my consent to all applicable items above • I am 18 years of age or older and purchasing this Service for my own personal use • I understand the wellness-only nature of Limitless Health UK’s Services and that no clinician-patient relationship is created • I understand that Limitless Health UK does not provide any service intended to diagnose, treat, monitor, or prevent disease • I confirm that I am participating voluntarily and of my own free will • I understand that I have had the opportunity to ask questions before providing consent, and that I can contact Limitless Health UK directly should I wish to do so • I understand that any actions I take based on Limitless Health UK’s guidance are undertaken entirely at my own discretion and risk • I understand that I may withdraw my consent at any time before my sample has been posted, by contacting Limitless Health UK, and that once posted, laboratory processing cannot be stopped • All information I have provided is accurate and complete • I acknowledge that this agreement forms part of a legally binding contract between myself and Limitless Health UK My purchase acts as my digital signature and constitutes my agreement to the terms of this form. |
5. Your Right to Withdraw Consent
You have the right to withdraw your consent at any time before your sample has been posted to the laboratory, by contacting Limitless Health UK using the details on our website. Once your sample has been posted to the laboratory, processing of that sample is outside Limitless Health UK’s control and cannot be stopped.
For PT and coaching services, you may withdraw your consent at any time before a session has commenced. Where a session or subscription package has already begun, our Cancellation and Refund Policy will apply.
Withdrawal of consent does not affect the lawfulness of any processing carried out prior to withdrawal.
