Consultation Agreement

To be completed before each wellness consultation or check-in session

Please read this agreement carefully before your consultation or check-in. By signing below, you confirm that you understand and agree to the terms set out in this document. This agreement should be read alongside your previously completed Informed Consent & Agreement Form.

1. Session Details

2. Scope of This Session

This is a wellness consultation or check-in only. It is not a medical appointment. No clinical assessment, diagnosis, treatment plan, or prescription will be provided at any point.

Limitless Health UK does not provide any service that is intended to diagnose, treat, monitor, or prevent disease. Nothing in this session creates a clinician-patient relationship or any regulated clinical duty of care.

During this session, your wellness consultant may:

Discuss the information presented within your test report (where applicable)
Review self-reported weight, urine, or symptom data you have provided (where applicable)
Provide general lifestyle, wellness, and supplement guidance informed by your report or data
Discuss general topics relating to sleep, hydration, stress, training, and nutrition

Your wellness consultant will not:

Diagnose any medical or health condition
Interpret results in a clinical or diagnostic manner
Provide clinical assessment, medical opinion, or treatment plan
Prescribe any medication, supplement, or clinical intervention
Assess fitness to compete or provide medical clearance of any kind
Act in the capacity of a clinician, doctor, or regulated healthcare professional

Any discussion of test results or monitoring data is based solely on the report or data provided. Limitless Health UK does not verify, validate, or clinically interpret these results. Any reliance placed on third-party test results is undertaken entirely at your own discretion and risk.

Any guidance or recommendations provided during this session is general in nature. Any decisions you make based on that guidance are made entirely at your own discretion and risk.

If any concern about your health arises during this session, you will be directed to seek appropriate medical attention from your GP or a qualified healthcare professional. No clinical guidance will be provided in that circumstance. You acknowledge that this session is not suitable for the assessment or management of any medical condition.

If you believe you are experiencing a medical emergency, you must contact 999 or attend A&E immediately. Do not wait for a wellness consultation or check-in.

This session is intended for general wellness discussion only and should not be used as a substitute for professional medical consultation.

3. Cancellation & No-Show Policy

Cancellations made more than 48 hours before your scheduled session: your session will be rescheduled at no additional charge.
Cancellations made with less than 48 hours notice: your session will be forfeited and no refund will be issued.
No-shows (failure to attend without notice): your session will be forfeited and no refund will be issued.
If you arrive late to your session, the session will still end at the originally scheduled time. The time lost cannot be recovered or refunded.
If Limitless Health UK needs to cancel or reschedule your session, we will notify you as soon as possible and offer an alternative at no additional charge.

4. Session Notes & Recording

Brief notes may be taken by your wellness consultant during this session. These notes are made solely for the purpose of delivering your wellness guidance and are not a clinical record.

Your session notes will be retained securely for up to 12 months from the date of this session to support any follow-up appointment you may book. They will be securely deleted at the end of that period in accordance with Limitless Health UK’s Privacy Policy.

You may request access to or deletion of your session notes at any time by contacting us.

Recording of sessions is not permitted without the explicit, prior agreement of both the client and the consultant. If you wish to record your session, you must notify your consultant before the session begins. Limitless Health UK reserves the right to decline a recording request.

5. Confidentiality

All information you share during this session will be treated as confidential. Information will not be shared with any third party except:

Where required by law or a regulatory authority
Where disclosure is necessary to protect your safety or the safety of others
With our secure communication or data storage providers, strictly for the purposes of service delivery and in accordance with our Privacy Policy

You are also asked to treat the content of your session as confidential and not to share proprietary guidance, methodology, or materials provided by Limitless Health UK without our prior written consent.

6. Payment

Payment for your consultation or check-in session is due in full at the time of purchase, prior to the session taking place. Sessions will not proceed where payment has not been received.

In the event of a dispute relating to payment, please contact us directly. Our full Cancellation and Refund Policy, available on our website, applies to all sessions.

7. Session Conduct

You agree to:

Attend your session at the scheduled time
Provide accurate and honest information during your session
Engage respectfully with your wellness consultant

Limitless Health UK reserves the right to terminate a session at any time where:

The service is being misused or used for a purpose outside its intended wellness scope
A client requests clinical advice, diagnosis, or treatment beyond the scope of the service
A client behaves in an abusive, threatening, or inappropriate manner

This Agreement, together with Limitless Health UK’s Terms and Conditions of Service, Privacy Policy, Medical Disclaimer, and Informed Consent & Agreement Form, forms part of the overall agreement between you and Limitless Health UK.

8. Limitation of Liability

Limitless Health UK’s total liability in connection with this session shall not exceed the amount paid by you for the session. We are not liable for any indirect, consequential, or special loss arising from your use of or reliance on guidance provided.

We do not guarantee any specific outcome or improvement from this session. Results will vary depending on individual factors including genetics, lifestyle, environment, and adherence to recommendations. You acknowledge that you are responsible for seeking independent medical advice where appropriate.

Nothing in this Agreement limits or excludes our liability for death or personal injury caused by negligence, fraud, or any other liability that cannot be excluded under applicable law.

9. Governing Law

This Consultation Agreement is governed by and construed in accordance with the laws of England and Wales.

10. Declaration & Digital Signature

DECLARATION By providing my full name and purchasing from Limitless Health UK, I confirm that: • I confirm that I have read and understood this Consultation Agreement in full • I understand and accept the wellness-only scope of today’s consultation or check-in • I agree to the cancellation and no-show policy set out in Section 3 • I consent to brief notes being taken during my consultation for the purpose of delivering my wellness guidance • I understand that my consultation notes will be retained for up to 12 months and then securely deleted • I confirm that no recording of this consultation will take place without the explicit agreement of both parties • I understand that information discussed during my consultation will be treated as confidential • I confirm that all information I provide during this consultation will be accurate and complete • I understand that if any concerns arise during this consultation I will be directed to seek appropriate medical attention • I confirm that I am participating voluntarily and of my own free will • I understand that any decisions I make based on guidance provided are made entirely at my own discretion and risk • I acknowledge that this agreement forms part of a legally binding contract between myself and Limitless Health UK My purchase acts as my digital signature and constitutes my agreement to the terms of this form.
